The route from Heathrow to Marylebone is one of the most straightforward in London: out of the airport’s northern side, east along the A40, onto the elevated Westway and down onto the Marylebone Road. The people who make it, though, are varied. Patients and their families come for appointments in the Harley Street medical district. Visitors stay in the hotels around Baker Street and Portman Square. And some are heading for Marylebone station, to catch a Chiltern train to Oxford, Bicester or Birmingham.

This guide covers each of those journeys, with the timings, the drop-off points and the Congestion Charge, which applies to some Marylebone addresses and not others.

This guide explains how a Heathrow to Marylebone transfer works from start to finish: the road, the timing and when the alternatives make more sense.

About 17 miles door to door. The rush-hour bar includes a standard allowance for London traffic; chauffeurs pick the line on the day.

How long does the drive take?

About 38 minutes off-peak, and about 55 minutes on a weekday morning, when the A40 inbound and the Marylebone Road both slow down. The Westway itself usually keeps moving; the queues build at the junctions where it meets the local roads, particularly where it comes down at Paddington.

An arrival before 07:00 is the quickest, often around half an hour. An arrival on a Friday evening is also usually easy inbound, because most traffic is heading out of London.

Arriving for a clinic appointment?

Many overseas patients land at Heathrow on the day of an appointment, or the day before. If the patient needs help moving through the airport, arrange special assistance with the airline before travel; it is free, and staff will bring the passenger from the aircraft to the arrivals hall, where our chauffeur will be waiting.

Tell us the clinic and the appointment time. For a same-day appointment, we suggest landing at least three hours before it, to allow for immigration and the drive. The chauffeur will take you straight to the clinic door, and if there is time to spare, can take the luggage on to your hotel so you arrive at the appointment unburdened.

Travelling with a wheelchair or medical equipment?

Tell us when you book. Folding wheelchairs, walking frames and portable medical devices fit in most of our cars, but we will choose the vehicle and the seating to make getting in and out easy. The chauffeur will allow extra time at both ends, help with the transfer from the wheelchair to the seat if you wish, and stow the equipment carefully. If a companion needs to sit beside the patient, the V-Class gives more room to do so comfortably.

Heading for a Chiltern train at Marylebone station?

Marylebone station is the London terminus for Chiltern Railways, with trains to Oxford, Bicester, Warwick, Birmingham and the Chiltern towns. Visitors often fly into Heathrow and continue by train, particularly to Oxford or to the Bicester Village outlet shopping centre.

The station is small and friendly compared with the big termini. The chauffeur will drop you at the entrance on Melcombe Place and help with bags. For a reserved train, allow at least two hours after a long-haul landing.

Which part of Marylebone is in the Congestion Charge zone?

The southern part. Everything south of the Marylebone Road, including Harley Street, Marylebone High Street and Portman Square, is inside the zone, so an arrival there between 07:00 and 18:00 on a weekday, or between 12:00 and 18:00 at weekends and on bank holidays, incurs the £18 daily charge. Marylebone station, Lisson Grove and the streets north of the Marylebone Road are outside it. Our quote is based on your exact address and includes the charge only when it applies.

What about the hotels around Baker Street?

The hotels around Baker Street, Portman Square and Dorset Square have set-down points at their entrances, and the chauffeur will pull up at the right door. The streets in between are one-way and busy with deliveries in the morning, so the chauffeur may approach from a side road. For serviced apartments in the area, give us the building name and any key collection instructions.

Car or train from Heathrow?

There is no direct train. The options are the Heathrow Express to Paddington and a short taxi or the Bakerloo line, or the Elizabeth line to Paddington or Bond Street and a change. With a small bag, both are workable. For a patient, a family or anyone with real luggage, the car from the terminal kerb to the door is simpler, and on this route it is usually as quick.

Where will the chauffeur meet you?

In the arrivals hall of your terminal, with a board showing your name. At Terminal 5, where many flights from North America land, the hall is long and open, and the chauffeur will stand near the central exit from customs. If special assistance staff are bringing you through, the chauffeur will meet you at the assistance point instead; tell us when you book.

What about the return journey?

Marylebone to Heathrow is a quick run outbound on the Westway, except on weekday evenings from about 16:00. For a long-haul flight, allow about three hours from pickup to departure. For patients leaving after treatment, allow a little more, so nothing is rushed. Heathrow’s £7 drop-off charge is included in the price.
